Abstract   Choriocarcinoma is the malignant tumor arising from fetal trophoblast and metastasized to the lung, mainly. Among the other distant metastatic sites, brain is the most serious and highly malignant form. Between 1973 and 1992, 701 patients received chemotherapy for gestational trophoblastic tu- mors in the department of obstetrics and gynecology, Catholic University Medical College. A retrospective analysis has been performed of patients treated for brain metastasis of 43 patients. The results were as follows; 1) the mean age on admission was 33.8 years(range 19-57, median 31). 2) Most of antecedent pregnancy was not hydatidiform mole. 3) There appears to be a tendency for metastasis to occur frequently in the left hemisphere. The parietofrontal area was the most affected area. 4) Headache associated with nausea and vomitting was the most common symptom. The next was hemiparesis and were fallowed visual disturbance and seizure. 5) Most of the patients had lung metastasis(84%) but, there was only brain metastasis in 15.9 % of patients. 6) The most effective treatment was the combination of systemic chemotherapy and intrathecal injection with or without craniotomy and intrathecal injection with radiation therapy was the worst. 7) The most common cause of death was intracranial hemorrhage and the next was infection.
